The Assertive Outreach Team aims to offer users an intensive, but flexible, service user led service. We will work within the guidelines of a Care Programme Approach, assessing needs and strengths, care planning and ensuring access to a range of services.
The Assertive Outreach Team will advocate on behalf of the service users who are unwilling or unable to engage and have a severe and enduring mental illness. It will aim to provide the best quality services to those in need, in a framework of mutual co-operation and respect.
It will work within the guidelines and values of the Foundation Trust Equal Opportunities Policy recognising the impact of stigma and discrimination on particular diverse groups.
The Assertive Outreach Team is committed to working in a multi-disciplinary way providing holistic services encompassing social, psychological and medical interventions to meet individual need. It will strive to raise the quality of life for clients and their families to maximise their potential and increase their life chances.
Who the service is for:
Dual Diagnosis - people with substance misuse and mental illness. Service exclusively for people on enhanced CPA. Referral from Specialist Mental Health Service only.
