Future Matters - Planning for the Last Years of Life

What do we mean by Planning for the Last Years of Life?
Planning for the last years of life (also referred to as end of life) can involve any, or all, of the following:

·  Planning for future care and support by completing an Advance Care Plan*. 

·  Discussing plans with loved one(s) and those who are responsible for your health and social care;

·   Making a will;

·   Organising a Power of Attorney;

·    Recording funeral wishes ;

·    Registering as an organ donor.

What can a Future Matters Volunteer do?
·     Support someone to start planning for their future care and support by helping them to complete an Advance Care Plan;

·     Support someone to discuss their plans with those who are important to them and those who are responsible for their health and social care;

·     Provide information about the benefits of writing a Will and support to find a Will writing service;

·     Provide information about Powers of Attorney and why they are needed;

·     Support someone to record their funeral wishes;

·     Signpost to other appropriate services.

What is outside of the Future Matters role?
·     Providing emotional, spiritual or practical support to someone who has had a terminal diagnosis and/or who is dying;

·     Supporting someone who has been bereaved;

·     Giving medical advice or information;

·     Giving legal or financial advice.

How can I make a referral?
You can book a 1:1 confidential appointment with a Future Matters Volunteer.
